Sažetak

 

 

Ankilozirajući spondilitis je autoimuna zapaljenska bolest koja uglavnom uključuje zglobove kičme, sakroilijačne zglobove i njihova susedna meka tkiva, kao što su tetive i ligamenti. U naprednim slučajevima, ova upala može dovesti do fibroze i kalcifikacija, što rezultira gubitkom fleksibilnosti i fuzijom kičme, koja liči na „bambusov štap” sa nepokretnim položajem. Glavne kliničke manifestacije uključuju bol i progresivnu ukočenost aksijalnog skeleta, kao i inflamaciju perifernih zglobova. Pored toga, postoje vanzglobne manifestacije, kao što su akutni prednji uveitis i inflamatorna bolest creva. Uprkos svim otkrićima, mehanizmi u osnovi bolesti nisu u potpunosti shvaćeni i nove informacije u vezi sa patogenezom i načinom lečenja su i dalje u procesu otkrivanja. Razumevanje kvaliteta života je važno za poboljšanje simptoma, pomoć, negu i rehabilitaciju pacijenata. Kvalitet života se takođe koristi za identifikaciju opsega problema koji mogu uticati na pacijente. U okviru ankilozirajućeg spondilitisa, pokazalo se da su fizičko funkcionisanje i aktivnost bolesti povezani i sa fizičkim i sa mentalnim aspektima kvaliteta života. Pacijenti često prijavljuju visoke nivoe bola i umora, poremećaj sna, depresiju i anksioznost.

Abstract

 

Ankylosing spondylitis is an autoimmune disease that mainly involves the joints of the spine, the sacroiliac joints and their adjacent soft tissues, such as tendons and ligaments. In advanced cases, this inflammation can lead to fibrosis and calcification, resulting in loss of flexibility and fusion of the spine, resembling a „bamboo stick” with an immobile position. The main clinical manifestations include pain and progressive stiffness of the axial skeleton, as well as peripheral joint inflammation. In addition, there are extra-articular manifestations, such as acute anterior uveitis and inflammatory bowel disease. Despite all the discoveries, the underlying mechanisms of the disease are not fully understood and new information regarding pathogenesis and treatment methods are still being discovered. Understanding quality of life is important for symptom improvement, assistance, care and rehabilitation of patients. Quality of life is also used to identify the range of problems that may affect patients. Within ankylosing spondylitis, physical functioning and disease activity have been shown to be associated with both physical and mental aspects of quality of life. Patients often report high levels of pain and fatigue, sleep disturbance, depression and anxiety.
