Sažetak

 

 

Uvod. Reumatoidni artritis (RA) je hronično zapaljensko autoimuno reumatsko oboljenje, a cilj njegovog lečenja je dovođenje bolesti u stanje remisije ili nisku aktivnost. Procena aktivnosti bolesti kod obolelih od RA je važna zbog praćenja toka bolesti i efekta terapije. Cilj ovog istraživanja je ispitati nivo aktivnosti bolesti kod pacijenata sa RA lečenih biološkom terapijom. Materijal i metode: Prospektivnom studijom preseka je obuhvaćeno 56 bolesnika oba pola, koji boluju od reumatoidnog artritisa i lečeni su biološkim lekom koji modifikuje tok bolesti u Specijalnoj bolnici za reumatske bolesti Novi Sad. Ispitanici su popunjavali opšti upitnik, koji je sastavljen od samog istraživača, i sadrži demografske podatke, podatke bolesti i terapiji. Za procenu aktivnosti bolesti korišćeni su sledeći indeksi: Disease Activity Score 28 (DAS28), Clinical Disease Activity Index (CDAI) i Simple Disease Activity Index (SDAI). Statistička obrada i analiza sprovedena je u statističkom paketu SPSS verzija 24. Rezultati: Rezultati merenja nivoa aktivnosti bolesti pokazali su da su DAS28 i CDAI u statistički značajnoj vezi sa radnim statusom (p˂0,01). SDAI je u statistički značajnoj vezi sa polom ispitanika (p˂0,05) i radnim statusom (p˂0,01). Podaci o bolesti i terapiji nisu u statistički značajnoj vezi sa DAS28. Dužina trajanja terapije biološkim lekom je u statistički značajnoj vezi sa CDAI (p˂0,01) i SDAI (p˂0,05). Zaključak: Radni status pacijenata obolelih od RA koji se leče biološkom terapijom utiče na nivo aktivnosti bolesti, odnosno viša aktivnost bolesti je kod onih koji su nezaposleni i pacijenti koji se duže leče biološkom terapijom su u remisiji ili niskoj aktivnosti bolesti.

Abstract

 

Introduction. As rheumatoid arthritis (RA) is a chronic inflammatory autoimmune rheumatic disease, the aim of any prescribed treatment is to achieve remission or low disease activity. Disease activity assessment in patients with RA is important for monitoring the course of the disease and determining the therapy effectiveness. The aim of this research was to examine the level of disease activity in RA patients undergoing biological therapy. Material and methods: This prospective cross-sectional study included 56 RA patients of both sexes receiving a biological drug that modifies the disease course all of whom were treated at the Special Hospital for Rheumatic Diseases in Novi Sad. The participants completed a general questionnaire developed by the researcher for the purpose of this investigation to obtain their key demographic data, as well as information about the disease and therapy. The disease activity was assessed via Disease Activity Score 28 (DAS28), Clinical Disease Activity Index (CDAI), and Simple Disease Activity Index (SDAI). Statistical processing and analyses were carried out in the statistical package SPSS version 24. Results: Statistically significant relationships between DAS28 and CDAI scores and employment status (p˂0.01), as well as SDAI scores and gender (p˂0.05) and employment status (p˂0.01) were noted. While the disease and therapy data were not statistically significantly related to DAS28 scores, a statistically significant relationship was established for the biological drug therapy duration with CDAI (p˂0.01) and SDAI (p˂0.05). Conclusion: In RA patients receiving biological therapy, being unemployed was associated with higher disease activity, while longer therapy duration was more likely to result in disease remission or low activity.
