Sažetak

 

 

Uvod: Reumatoidni artritis (RA) je hronična, sistemska, autoimuna, inflamatorna bolest od koje u većem procentu obolevaju žene.Pored svih dostupnih terapijskih modaliteta koji se sa velikim uspehom koriste u reumatološkoj praksi, čime se znatno popravio i ishod lečenja reumatoloških pacijenata,zamor je i dalje propratni simptom RA. Cilj rada: Ispitati faktore rizika koji utiču na zamor kod pacijenata sa reumatoidnim artritisom. Materijal i metode: Retrospektiva prospektiva studija preseka je obuhvatila 39 pacijentkinja prosečne starosti 60,56 godina koje boluju od reumatoidnog artritisa i lečene su u bolničkim uslovima u Specijalnoj bolnici za reumatske bolesti Novi Sad. Analizirani su Functional Assessment of Chronic Illness Therapy scale, Vizuelna analogna skala, snaga stiska šake pomoću dinamometra i potencijalni faktori rizika za nastanak zamora, kao što su: godine života, indeks telesne mase, dužina trajanja zapaljenske reumatske bolesti, godina ulaska u menopazu, bračno stanje, mesto življenja, podatak gde žive (kuća/stan), sa kim žive, obrazovni nivo, radni odnos, radno mesto i intenzitet fizičke aktivnosti.Dobijeni podaci su obrađeni u statističkom paketu SPSS ver. 25 (Statistical Package for the Social Sciences) for Windows. Rezultati: Prosečna starost ispitanika je 60,56 godina. Prosečna dob ulaska u menopauzu iznosi 48,64 godine, a trajanje zapaljenske reumatske bolesti 12,5 godina. Zamor je u statistički slaboj pozitivnoj korelaciji sa godinama života (r = 0,178, p = 0,277), umerenoj pozitivnoj korelaciji sa telesnom masom (r=0,312, p=0,053), a u statistički značajnoj vezi sa jačinom bola (r=0,435, p=0,006). U odnosu na FACIT- Fatigue skalu, prosečna vrednost zamora je bila više kod ispitanica koje su u braku (28,26), žive u prigradskom naselju (33), koje su u radnom odnosu ili u penziji, međutim nije pronađena statistički značajna razlika sa zamorom (p>0,05). Kada je u pitanju nivo fizičke aktivnosti nije pronađena statistički značajna povezanost sa zamorom (p = 0,067), iako su ispitanice sa niskim nivoom aktivnosti imale nešto višu prosečnu vrednost zamora (30,4). Zaključak: Starija životna dob, povećana telesna masa, niži nivo fizičke aktivnosti, radno angažovanje, kao i bračno stanje su prepoznati kao prediktori za nastanak zamora.

Abstract

 

Introduction: Rheumatoid arthritis (RA) is a chronic, systemic, autoimmune, inflammatory disease that predominantly affects women. Despite the significant therapeutic modalities available and successfully employed in rheumatological practice, resulting in substantial improvements in the outcome of rheumatology patients, fatigue remains an enduring accompanying symptom of RA. The aim: To investigate risk factors influencing fatigue in patients with rheumatoid arthritis. Materials and methods: A retrospective prospective cross-sectional study encompassed 39 female patients with an average age of 60.56 years diagnosed with rheumatoid arthritis and treated in a hospital setting at the Special Hospital for Rheumatic Diseases in Novi Sad. The study analyzed the Functional Assessment of Chronic Illness Therapy scale, Visual Analog Scale, hand grip strength using a dynamometer, and potential risk factors for fatigue, such as age, BMI, duration of inflammatory rheumatic disease, age at menopause onset, marital status, place of residence, housing type (house/apartment), living arrangements, educational level, employment status, work position and intensity of physical activity. Data were processed using the statistical software package SPSS ver. 25 (Statistical Package for the Social Sciences) for Windows. Results: The participants' average age was 60.56 years. The average age of menopause onset was 48.64 years, and the average duration of inflammatory rheumatic disease was 12.5 years. Fatigue exhibited a statistically weak positive correlation with age (r = 0.178, p = 0.277), a moderate positive correlation with body mass index (BMI) (r = 0.312, p = 0.053), and a statistically significant association with pain intensity (r = 0.435, p = 0.006). Concerning the FACIT-Fatigue scale, the average fatigue score was higher among married participants (28.26) and those residing in suburban areas (33) and who were either employed or retired; however, no statistically significant difference was found in relation to fatigue (p > 0.05). Regarding the level of physical activity, no statistically significant correlation was observed with fatigue (p = 0.067), even though participants with a low activity level had a slightly higher average fatigue score (30.4). Conclusion: Advanced age, increased body mass, lower physical activity levels, employment status, and marital status have been identified as predictors for the development of fatigue.
