Sažetak

 

 

Uvod: Lumbalni sindrom se definiše kao bol i nelagodnost lokalizovana ispod donjeg kostalnog ruba i iznad inferiornog glutealnog pregiba, sa ili bez propagacije bola u nozi, što može dovesti do onesposobljenosti ovih pacijenata. Cilj: Ispitati efekat Horizontalne terapije na bol i onesposobljenost kod pacijenata sa lumbalnim sindromom. Materijal i metode: Prospektivna studija preseka je obuhvatila 33 pacijenata oba pola, starosti ≥ 30 godina, obolelih od lumbalnog sindroma, sa pristunim bolom u lumbalnom delu kičme,  lečenih u Specijalnoj bolnici za reumatske bolesti u Novom Sadu. Istraživanje je sprovedeno nakon odluke Etičkog odbora (broj: 14/34-9/1-23). Svi pacijenti su potpisali informisani pristanak o učešću u istraživanju. Ispitanici su lečeni Horizontalnom terapijom tokom deset terapijskih dana i kineziterapijom istog protokola.  Svi ispitanici su popunili upitnik o sociodemografskim podacima, sastavljenim od strane ispitivača. Pre i posle terapije, svi ispitanici su popunili Roland-Morisov upitnik o onesposobljenosti, Vizuelno analognu skalu i izmeren im je modifikovani modifikovani Šober test (cm). Rezultati: Prosečna starosna dob ispitanika je bila 56,45 ± 8,35 godina sa predominacijom ženskog pola (78,8%). Rezultati između dva merenja modifikovanim modifikovanim Šober testom su pokazali da je pokretljivost lumbalnog dela kičme bila bolja nakon terapije (t=-2,43, df=31, p=0,021). Nivo bola je statistički značajno smanjen (p≤0,001) nakon terapije: pre terapije M=6,94, nakon terapije M=5,09. Utvrđeno je statistički značajno smanjenje onesposobljenosti upoređivanjem vrednosti Roland Morisovog upitnika o onesposobljenosti nakon terapije u odnosu na početak tretmana (t=6,479, df=31, p≤0,001). Pre terapije prosečna ocena na Roland Morris upitniku iznosila je M=12,59, a nakon terapije M=9,31.  Najveći napredak ispitanici su ostvarili na ajtemu: ,,Zbog bola u leđima spavam manje nego inače” i to njih 33,3%. Zaključak: Horizontalna terapija uz kineziterapiju je dovela do smanjenja bola, povećane pokretljivosti lumbalnog dela kičme, smanjenog nivoa onespobljenosti i poboljšanja kvaliteta sna kod pacijenata sa dijagnozom lumbalnog sindroma.

Abstract

 

Introduction: Low back pain is defined as pain and discomfort localized below the low costal edge and  above the superior gluteal fold with or without leg pain propagation, which can cause disability in these patients. The Aim: To examine the effect of Horizontal therapy on pain and disability in patients with low back pain. Material and methods: A cross-sectional prospective study included 33 subjects of both sex, aged ≥ 30, suffering from low back pain with present pain in the lumbar spine, treated in a Special hospital for rheumatic diseases in Novi Sad. The study was implemented after the decision of an Ethics Committee (number: 14/34-9/1-23). All patients signed informative consent to participate in the study. The subjects were treated with Horizontal therapy and kinesitherapy of the same protocol for ten therapeutic days. All patients filled out a questionnaire of sociodemographic data, compiled by the examiner. Before and after therapy, patients filled out Roland-Morris Disability Questionnaire, Visual Analog Scale and were measured by the modified modified Schober test (cm). Results: The average age of subjects was  56,45 ± 8,35 with a predominance of the female gender (78,8%). Results between two measurements by modified modified Schober test showed that mobility of the lumbar spine is better after therapy (t=-2,43, df=31, p=0,021). The level of pain is statistically significantly reduced  (p≤0,001) after therapy: before therapy M=6,94, after therapy M=5,09. A statistically significant decrease in disability was found by comparison between two mean score values of the Roland Morris Disability Questionnaire before and after therapy (t=6,479, df=31, p≤0,001). Before therapy the average score was M=12,59, and after therapy was M=9,31. The best progress was found on item:,, Because of back pain, I sleep less than usual‘‘ with a percentage of 33,3%. Conclusion: Horizontal therapy with kinesitherapy leads to decrease pain, disability, and increased mobility of the lumbar spine and quality of sleep in patients with low back pain.
