Sažetak

 

Uvod: Tumori mokraćne bešike su značajan su uzrok morbiditeta i mortaliteta kod čoveka i ubrajaju se među najvažnije neoplazme ljudskog organizma, sa većom incidencom kod muške populacije. Patohistološki 99% dignostikovanih neoplazmi je karcinom porekla urotela Većina tumora patohistološki se klasifikuju kao papilarni urotelni karcinomi. Stepen malignosti može da varira od tumora niskog stepena do visokog stepena malignosti. Korelacija različitih patohistoloških tipova i podtipova karcinoma mokraćne bešike sa patološkim stadijumom bolesti. Materijal i metode: Studija je bila retrospektivna, obuhvatila je desetogodišnji period i 249 pacijenata, kod kojih je nakon radikalne cistektomije izvršena patohistološka analiza materijala, u periodu od početka 2009. do kraja 2019. godine, uz upotrebu protokola Američkog koledža patologa iz 2019. godine. U studiji su analizirani pol i starost pacijenata, histološki tip i veličina tumora, mikroskopsko širenje, histološki gradus, konfiguracija tumora, prisustvo limfovaskularne invazije, zahvatanje hirurških margina, stadijum bolesti i komorbiditeti pacijenata. Rezultati: Najveći procenat patohistološki dijagnostifikovanih tumora su urotelni karcinomi (96,4%), a znatno manji broj pripada skvamocelularnom karcinomu (1,19%), neuroendokrinim timorima (1,98%)  i adenokarcinomu (0,4%). U nalazima dominira visok stadijum bolesti, Stage III (67%)  i  Stage IV(7%). Za korelaciju različitih histoloških tipova i podtipova karcinoma mokraćne bešike sa patološkim stadijumom bolesti korišćen je χ2 test, pokazuje da postoji statistički značajna razlika u prognozi različitih histoloških tipova i podtipova karcinoma mokraćne bešike u odnosu na patološki stadijum bolesti (p<0,05). Zaključak: Biološko ponašanje različitih histoloških tipova i podtipova karcinoma mokraćne bešike određeno je patološkim stadijumom bolesti. Najagresivniji tumori su neuroendokrini i skvamocelularni karcinom, a nabolju prognozu za preživljavanje ima čist urotelni karcinom.

Abstract

 

Introduction: Urinary bladder tumors are a major cause of morbidity and mortality in humans and are among the most important neoplasms of the human organism. They are more common among men. Most tumors are patohistologically classified as papillary urothelial carcinomas (99%). The degree of malignancy may vary from low-grade malignancy tumor to high-grade malignancy. Correlation of different pathohistological types and subtypes of the urinary bladder carcinoma with pathological stage of illnes. Material and Methods: The study was retrospective and included 249 patients in whom, after radical cystectomy, a pathohistological analysis of the material was carried out between the beginning of 2009 and the end of 2019 using the protocol of the American College of Pathologists from 2019. The study analyzed the sex and age of patients, the histological type and tumor size, microscopic tumor extension, histological grade, tumor configuration, the presence of lympho - vascular invasion, presence of surgical margin, disease stage and patients' comorbidities. Results: The highest percentage of pathohistologically diagnosed tumors are urothelial carcinomas (96,4%), significantly smaller number belongs to squamocellular carcinomas (1.19%) and  neuroendocrine tumors (1.98%) and adenocarcinoma (0.4%). The findings are dominated by a high stage of the disease, stage III (67%) / stage IV (7%). χ2 test was used to correlate different histological types and subtypes of urinary bladder cancer with pathological disease and shows that there is statistically significant difference in the prognosis of different histological types and subtypes of the bladder carcinoma in relation to the pathological stage of the disease (p <0.05). Conclusion: The biological behavior of different histological types and subtypes of the bladder carcinoma is determined by the pathological stage of the disease. The most aggressive tumors are neuroendocrine and squamocellular carcinoma, and the best prognosis for survival has pure urothelial carcinoma.
