Sažetak

 

 

Uvod: Mezonefrični (Volfovi) ostaci predstavljaju retke embriološke strukture koje mogu perzistirati u ženskom genitalnom traktu, najčešće u stromi grlića materice. Njihovo pravilno prepoznavanje ima dijagnostički značaj, jer se histološki mogu zameniti sa žlezdanim neoplazmama grlića materice. Prikaz slučajeva: U ovom radu predstavljamo dva slučaja mezonefričnih ostataka identifikovanih kod pacijentkinja srednje životne dobi kojima je urađena konizacija grlića materice u okviru dijagnostičko-terapijskog postupka. Mikroskopski su u stromi grlića materice uočene tubularne strukture obložene kubičnim epitelom bez citološke atipije, sa luminalnim eozinofilnim materijalom. Sumnja na mezonefrične ostatke potvrđena je imunohistohemijskim profilom: PAX8+, GATA3+, CD10+, p16–, ER–, PR–. Zaključak: Iako predstavljaju benigni i obično slučajni nalaz, mezonefrični ostaci mogu histološki imitirati neoplastične procese. Imunohistohemijska analiza je od suštinskog značaja za njihovo tačno prepoznavanje i diferencijalnu dijagnozu u odnosu na adenokarcinome grlića materice. Svest o ovom entitetu doprinosi izbegavanju pogrešne dijagnoze i neadekvatnog terapijskog pristupa.

Abstract

 

Introduction: Mesonephric (Wolffian) remnants are rare embryological structures that may persist within the female genital tract, most commonly in the cervical stroma. Accurate recognition is diagnostically important since their morphology can mimic glandular neoplasms of the cervix. Case presentation: We present two cases of mesonephric remnants identified in middle-aged women who underwent cervical conization as part of diagnostic-therapeutic management. Histological examination revealed tubular structures lined by cuboidal epithelium without cytologic atypia, containing eosinophilic luminal material. Immunohistochemical staining confirmed the diagnosis, showing positivity for PAX8, GATA3, and CD10, and negativity for p16, ER, and PR. Conclusion: Although benign and usually incidental findings, mesonephric remnants can closely resemble neoplastic lesions. Immunohistochemistry plays a key role in distinguishing these benign inclusions from malignant glandular proliferations. Awareness of this entity is essential to avoid misdiagnosis and unnecessary treatment.
