Abstract

 

 

Parental instinct is manifested as a need to be a parent by taking care of the child during pregnancy and taking care of the child in the postnatal period. Sex drive without parental instinct is not sufficient to ensure the reproduction of the species. Infertility is defined as the inability of a couple to conceive after one year of coitus without the use of a contraceptive. The unequivocal connection between infertility and impairment of individual psychic stability can be hypothetically explained by numerous mechanisms: the perceived lack of realization on the personal and social level, the unsatisfactory vital instinctive needs, which psychodynamically gives space to generate suppressed neurotic conflicts, lack of fulfillment, continuous expectation with possible repeated disappoint­ments, sensitivity to numerous and sometimes unpleasant procedures, genuine or iatro­ge­nic hormonal imbalance, often significant material expenditures and other. A precise pathogenic mechanism that links stress with sterility is not yet proven, in this regard, significant research is being carried out that connects psychometric parameters with endocrinological laboratory parameters, primarily pituitary, adrenal, and gonadotrophic function. All findings suggest a need to reduce the stress of infertility treatment and to help patients cope with treatment and treatment failure. In this regard, we address the topics of psychotherapy treatment of infertility. The five most common approaches of individual counseling with people suffering from infertility are psychodynamic psycho­therapy, cognitive-behavioral treatment, strategic psychotherapy and focus on treatment, crisis intervention, and grief counseling. The aim of this paper is to review the current literature and scientific articles dealing with psychiatric, psychological and psychotherapeutic aspects of infertility and its treatment..

Sažetak:

 

Roditeljski instinkt se manifestuje kao potreba da se bude roditelj, kroz brigu o detetu tokom trudnoće i brigu o detetu u postnatalnom periodu. Seksualni nagon bez roditeljskog instinkta nije dovoljan da obezbedi reprodukciju vrste. Neplodnost se definiše kao ne­mogućnost bračnog para da zatrudni nakon godinu dana koitusa bez upotrebe kontra­ceptiva. Nedvosmislena veza između neplodnosti i narušavanja individualne psihičke stabilnosti može se hipotetički objasniti brojnim mehanizmima: uočenim nedostatkom reali­zacije na ličnom i društvenom nivou, nezadovoljenim vitalnim i instinktivnim potrebama, što psihodinamski stvara prostor za aktivaciju potisnutih neurotičnih konflikata; zatim nedostatkom zadovoljstva, kontinuiranim očekivanjima praćenim ponovljenim razo­čaranjima i neispunjenjem, pojavom pravih ili jatrogenih hormonskih disbalansa, kao i značajnim materijalnim izdacima, i drugim faktorima. Iako još uvek nije dokazan jasan patogeni mehanizam koji direktno povezuje stres sa sterilitetom, sprovode se značajna istraživanja koja povezuju psihometrijske parametre sa endokrinološkim laboratorijskim pokazateljima, pre svega funkcijama hipofize, nadbubrežnih žlezda i gonadotropne osovine. Svi nalazi ukazuju na potrebu za smanjenjem stresa izazvanog lečenjem neplodnosti i na važnost pružanja podrške pacijentima u suočavanju sa samim procesom lečenja i mogućnošću neuspeha. U tom kontekstu, posebna pažnja posvećuje se psihoterapijskim pristupima u tretmanu neplodnosti. Pet najčešćih oblika individualnog savetovanja obuhvataju: psihodinamsku psihoterapiju, kognitivno-bihejvioralni tretman, stratešku psihoterapiju sa fokusom na proces lečenja, kriznu intervenciju i savetovanje o tuzi. Cilj ovog rada je pregled aktuelne literature i naučnih članaka koji se bave psihijatrijskim, psihološkim i psihoterapijskim aspektima neplodnosti i njegovim lečenjem.
