Sažetak

 

 

Uvod: Neutrofilo-limfocitni odnos (NLR) i imunološki odgovor organizma na traumu se u mnogim istraživanjima pokazao kao prediktivni faktor nastanka hiperplazije neointime i kao loš prognostički faktor kod pacijenata sa bakterijemijom. Cilj i metod: Cilj istraživanja je bio da dovede u vezu morfološku strukturu karotidnog plaka verifikovanog doppler dijagnostikom sa signifikantnom morfološkom stenozom i NLR odnos u postoperativnom toku praćenja sa pojavom neointimalne hiperplazije. Operisano je 38 pacijenata sa signifikantnom karotidnom stenozom koji su bili selektovani u dve grupe prema strukturi plaka- grupa sa dominantno kalcifikovanim plakom i grupa pacijenata sa dominantno mekotkivnim plakom. Praćene su NLR vrednosti preoperativnog, prvog, trećeg, petog dana, mesec dana i tri meseca nakon operacije. Rezultati: Kod pacijenata u grupi sa pretežno kalcifikovnaim plakom je nakon trećeg meeca praćenja zabeleženo značajno povećanje neointimalnog kompleksa i značajno povećenje NLR odnosa. Zaključak: U obe grupe je prvog postoperativnog dana skoro identičan imunološki odgovor na endarterektomiju, dok nakon trećeg meseca praćenja u II grupi pacijenata postoji povećanje NLR odnosa što može korelirati sa zadebljanjem neointimalnog kompleksa.

Abstract

 

Introduction: The neutrophil-lymphocyte ratio (NLR) and the body's immune response to trauma have been shown in many studies to be a predictive factor for the development of neointimal hyperplasia and a poor prognostic factor in patients with bacteremia. Objective and Method: The aim of this study was to relate the morphological structure of carotid plaque, verified by Doppler diagnostics with significant morphological stenosis, to the NLR ratio in the postoperative follow-up period and the occurrence of neointimal hyperplasia. A total of 38 patients with significant carotid stenosis were operated on, and they were selected into two groups based on plaque structure: a group with predominantly calcified plaque and a group with predominantly soft tissue plaque. NLR values were monitored preoperatively, on the first, third, fifth days, one month, and three months after surgery. Results: In the group of patients with predominantly calcified plaque, a significant increase in the neointimal complex and a significant increase in the NLR ratio were recorded after three months of follow-up. Conclusion: On the first postoperative day, both groups exhibited an almost identical immune response to endarterectomy. However, after three months of follow-up, an increase in the NLR ratio was observed in group II patients, which may correlate with the thickening of the neointimal complex.
