Sažetak

 

 

Uvod. Tonzilektomija kao jedna od najčešćih procedura u dečijoj hirurgiji često se izvodi u kombinaciji sa adenoidektomijom. Smatra se da će adenotonzilektomije postoperativno subjektivno dati umeren do visok intezitet bola koji može da traje i do dve nedelje. Postoperativni bol je od velike važnosti kod dece zbog centralnog nervnog sistema i puteva za modulaciju bola, koji su u razvoju i samim tim imaju veću predispoziciju za fenomen hiperalgezije i nastanak hroničnog bola u odnosu na odrasle. Cilj. Ciljevi istraživanja su da se prikaže najčešća analgezija nakon adenotonzilektomije da li je postoperativan bol kupiran adekvatno  i učestalost komplikacija nakon adenotonzilektomije kod dece. Materijal i metode. Klinička retrospektivna studija sprovedena na Klinici za dečju hirurgiju u Novom Sadu u koju su uključena dece sa adenotonzilketomijom u periodu od  1. januara do 30. juna 2023.godine. Uvidom u medicinsku dokumentaciju uzeti su osnovni podaci, dijagnoza, komorbiditeti, ASA status, premedikacija, vrsta anestezije, dužina trajanja operacije i anestezije, procena bola korišćenjem VAS skale (0-10), vrsta analgezije, ukupna potrošnja analgetika, kao i postoperativne komplikacije. Rezultati. U istraživanju je učestvovalo 75 dece, 40 dečaka i 35 devojčica. Prosečna starost je bila 5,94±2,78  godine. Najčešće kombinacije analgetika bile su Paracetamol i Novalgetol (N = 21; 28,38%), Paracetamol i Trodon (N = 18; 24,32%). Najveća potrošnja je zabeležena za Paracetamol i Novalgetol. Prosečne vrednosti postoperativnog bola su bile niske (VAS do 2). Zaključak. Paracetamol i Novalgetol su najčešće korišćeni analgetici postoperativno, a VAS vrednosti jačine bola su ukazale na adekvatnu analgeziju nakon adenotonzilektomije.

Abstract

 

Introduction. Tonsillectomy, as one of the most common procedures in children's surgery, is often performed in combination with adenoidectomy. It is believed that postoperative adenotonsillectomy will subjectively cause moderate to high pain intensity that can last up to two weeks. Postoperative pain is of great importance in children due to the central nervous system and pathways for pain modulation, which are in development and thus have a greater predisposition to hyperalgesia and the development of chronic pain compared to adults. Aim: The research aims to show the most common analgesia after adenotonsillectomy, whether postoperative pain is adequately controlled, and the frequency of complications in children. Material and methods. A clinical retrospective study was conducted at the Children's Surgery Clinic in Novi Sad. By examining the medical documentation, basic data were taken, including diagnosis, comorbidities, ASA status, premedication, type of anesthesia, duration of surgery and anesthesia, pain assessment using the VAS scale (0-10), type of analgesia, total consumption of analgesics, as well as postoperative complications. Results. In the research were included 75 children, 40 of them were  boys and 35 girls . The average age was 5.94±2,78  years. The most common combinations of analgesics were Paracetamol and Novalgetol (N = 21; 28.38%), Paracetamol and Trodon (N = 18; 24.32%). The highest consumption was recorded for Paracetamol and Novalgetol. Average values of postoperative pain were low (VAS up to 2). Conclusion. Paracetamol and Novalgetol are the most commonly used analgesics postoperatively, and VAS pain intensity values indicated adequate analgesia after adenotonsillectomy.
