Sažetak

 

 

Uvod: Gastritis predstavlja zapaljenje želudačne sluznice i može biti izazvano različitim faktorima. Postavljanje dijagnoze gastritisa kod dece, prvenstveno se oslanja na patohistološki nalaz endoskopski dobijene biopsije sluznice želuca, pri čemu se skrining dece kojoj je potrebno uraditi gastroskopiju zasniva na kliničkoj slici.Cilj: Cilj ove studije jeste da se utvrdi povezanost simptoma i znakova gastritisa sa patohistološkim nalazom kod dece u odnosu na pol i uzrast ispitanika. Materijal i metode: U studiju su uključeni svi pedijatrijski pacijenti kojima je od 1.1.2019. do 1.1.2020. godine urađena endoskopska biopsija antralne sluznice želuca zbog postojanja nekog od simptoma i znakova gastritisa. Patohistološki podaci su semikvantitativno ocenjeni na osnovu Sidnejske klasifikacije gastritisa. Rezultati: Obrađeni su podaci o 147 pacijenata pedijatrijskog uzrasta. Gastritis je češće bio prisutan kod devojčica i dece starijeg školskog uzrasta. Dominantni simptomi gastritisa su bili: bol u trbuhu (59,2%), proliv i krv u stolici (po 27,9%). Inflamacija sluznice želuca kao patohistološki parametar ima visoku statistički značajnu povezanost sa anoreksijom kod dece mlađeg uzrasta i sa povraćanjem kod dece starijeg uzrasta, dok atrofija sluznice želuca ima visoku statistički značajnu povezanost kod dece mlađeg uzrasta sa usporenim rastom. Zaključak: Kod dece su bolovi u trbuhu najčešća indikacija za gastroskopiju, a potom slede proliv i krv u stolici. Kombinacija kliničkog i patohistološkog nalaza ostaje presudna za konačnu dijagnozu gastritisa kod dece.

Abstract

 

Introduction: Gastritis is an inflammation of the gastric mucosa and can be caused by various factors. The diagnosis of gastritis in children is primarily based on the pathohistological findings of the endoscopically obtained biopsy of the gastric mucosa, while the screening of children who need gastroscopy is based on the clinical picture. Goal: The aim of this study is to determine the association of symptoms and signs of gastritis with pathohistological findings in children in relation to gender and age of the subjects. Material and methods: The study included all pediatric patients who underwent endoscopic biopsy of the antral gastric mucosa from January 1, 2019, to January 1, 2020, due to the presence of one or more symptoms and signs of gastritis. Pathohistological data was semi-quantitatively assessed based on the Sydney classification of gastritis. Results: Data on 147 pediatric patients was processed. Gastritis occurs more frequently in girls and children of older school age. The dominant symptoms of gastritis were: abdominal pain (59.2%), diarrhea and blood in the stool (27.9% each). Inflammation of the gastric mucosa as a pathohistological parameter has a high statistically significant association with anorexia in young children and with vomiting in older children, while gastric mucosal atrophy has a high statistically significant association in young children with growth retardation. Conclusion: Abdominal pain is the most common indication for gastroscopy in children, followed by diarrhea and blood in the stool. The combination of clinical and pathohistological findings remain crucial for the final diagnosis of gastritis in children.
